Practice name, qualifications and registration
A medical letterhead usually names both the clinician and the practice, and the order tells the reader which one is writing. A consultant's referral letter leads with the clinician; a practice-wide notice leads with the practice. Post-nominal qualifications normally follow the name, and most clinicians show a professional registration number somewhere on the page.
Which registration number, and whether it is required at all, depends entirely on your regulator and country — so take the field as a place to put yours rather than as a rule about which one to use. The tax ID and registration number fields in Extras are generic text, so either can hold a medical registration number.
Referrals, prescriptions and notes are different documents
One letterhead rarely covers everything a practice sends. A referral letter is correspondence to a colleague and benefits from a full header with complete contact details. A prescription is a controlled document with requirements set by your regulator, and is not something to improvise from a general template. Appointment letters and recall notices are routine mail where a lighter header leaves more room for the message.
The practical approach is to design the referral version first, then save a lighter variant for routine post. Both live in the Saved panel and switch in a click.
